The Island Boards of Health Are Sponsoring a Flu Clinic

Saturday October 20th From 10:00 am To Noon In The MV High School Cafeteria

If you wish to receive a flu shot please print the attached (click) vaccine administration record and (click) screening questionnaire and bring the completed forms with you to the high school.  Please also remember to bring your health insurance card for verification.   Although insurance will be billed if possible, there is no charge for the vaccine and nobody will be turned away regardless of their insurance status.  Hope to see you on October 20th.
